If you wish to register with the practice you will need to come into the surgery to collect the paperwork you are required to complete. Alternatively, you can print off both sets of paperwork by clicking on the links below. You will need to complete the Pre-registration Form, sign it and bring it to the surgery. If you submit the Pre-registration Form electronically rather than printing it, we still require you to come into the surgery to sign the form, and also bring in your completed New Patient Questionnaire. All patients at Trent View Medical Practice have a named GP.
In addition, we also need you to provide proof of identification (preferably photo ID) such as a passport or drivers licence. You will also need to provide proof of your address, such as a recent utility bill.

Summary Care Record - your emergency care summary
The NHS in England is introducing the Summary Care Record, which will be used in emergency care situations.
The record will contain information about any medicines you are taking, allergies you suffer from, and any bad reactions to medicines you have had, to ensure those caring for you have enough information to treat you safely. This information will be available to authorised healthcare staff providing your care anywhere in England.
